All Electronic forms will require you to login with your CAT ID and passphrase so verification can be done for accessing the form.

  • Graduate students completing a committee approval form will have access to complete their own committee form by using their CAT ID and passphrase to login, no security request is needed.
  • University employees need to submit a Security Role Request for access to the GC01 role (Web Custom)

Graduate Assistantship Application

This application form should be completed and submitted by a qualifying graduate student applying for a graduate assistantship position.

Graduate College Tuition Scholarship Application

This application form should be completed and submitted by a qualifying graduate student applying for a Graduate College Tuition Scholarship with their academic department.

University Departmental forms

Extra Graduate College Support Request Form

This form should be completed and submitted by any UNI department requesting Graduate College funding support for Graduate Assistantships and/or Graduate College Tuition Scholarships.

Under-Represented Minority Support Request Form 

This form should be completed and submitted by graduate academic programs requesting Graduate College funding support for a qualifying under-represented minority graduate student for a Graduate Assistantship and/or Graduate College Tuition Scholarship.

Faculty Awards

Distinguished Scholar Award Nomination

Nominee must be a UNI faculty member whose professional reputation is national with a long and distinguished record of publications, exhibits and/or creative activities.

James F. Lubker Faculty Research Award Nomination

Nominee must be a UNI faculty member with five years of service at UNI and exceptional original contributions to research and scholarship, including the performing arts.

Outstanding Graduate Faculty Teaching Award Nomination

Nominee must be a UNI full-time tenured or tenure-track member of the regular graduate faculty for a minimum of three years and has displayed outstanding teaching in the graduate program.

Professional Development Assignment Application

Applicants must be UNI tenured faculty or probationary faculty after a successful third-year review. Applicant must hold a full-time (9 month) appointment at the time of application, during the assignment period and for one year following the assignment.

Summer Fellowship Application

Applicants must be UNI faculty holding a full-time (9 month) tenured or tenure-track appointment at the time of application.

Graduate Faculty Forms

Paw print Associate Graduate Faculty Status Form

This form should be completed for individuals who are not part of the regular graduate faculty at UNI in which you want to request they teach graduate-level course(s). 

Paw print Graduate Faculty Nomination Regular Membership

This form should be completed for a faculty member that meets educational requirements to teach graduate level courses.

Graduate Transfer Credit Form

Graduate Transfer Credit Evaluation Form

Graduate credit earned at other regionally accredited colleges and universities may be accepted to a maximum of one-third of the minimum number of hours required by a particular UNI graduate degree program, but doctoral programs may allow a smaller fraction of transfer hours. Credits earned from other institutions will not satisfy the minimum 6000-level course requirement (or 6000/7000-level requirement for doctoral degrees). Transfer courses taken after degree admission will not be processed or approved for inclusion in the program of study until official transcripts for these courses are filed with the Office of Admissions, and a Graduate Transfer Credit Evaluation Form is completed.

This form should be completed by the academic department when a student passes their comprehensive examination.

Paw print Report of Non-Thesis Paper Approval

This form should be completed by the academic department when a graduate student has submitted an approved non-thesis paper.

Student Dissertation Forms

Doctor of Education

Paw print EdD Dissertation Committee Appointment Form

EdD students should complete and submit this form as soon as their doctoral committee has been formed. Final approval notification of doctoral committee by the Graduate College will be sent via email.

Dissertation Submission Form

EdD students should complete and submit this form along with a final copy of their dissertation to the Graduate College.

EdD Dissertation Approval Form

EdD students should complete and print this form and take along to their dissertation defense.  When all signatures have been obtained indicating final approval of the dissertation, the form should be submitted to the Graduate College as part of the dissertation final submission materials. The Graduate Dean will sign the form after the dissertation has been approved by the Graduate College.

Doctor of Industrial Technology

Paw print DIT Faculty Advisory Committee Appointment Form

DIT students should complete and submit this form as soon as their doctoral committee has been formed. Final approval notification of doctoral committee by the Graduate College will be sent via email.

Dissertation Submission Form

DIT students should complete and submit this form along with a final copy of their dissertation to the Graduate College.

DIT Dissertation Approval Form
