*Measles, Mumps, and Rubella (MMR)***

*Immunization to measles, mumps, and rubella (MMR) is a requirement for registration. To comply all students must provide immunization records documenting proof of TWO measles, mumps and rubella (MMR) vaccinations.* Please complete the enclosed Health Requirement Immunization Verification form and include your immunization documentation. * *Note all the immunization dates must include month, day and year. Mandatory health requirement forms for all new students must be on file and in compliance by the tenth day of class during the first term enrolled. If students are not in compliance a hold will be placed on the students’ accounts preventing them from registering for future classes and a $30 late fee will be assessed. *Meningitis*

Meningococcal disease (meningitis) is a potentially life-threatening contagious bacterial infection. College students are at an increased risk to contract meningococcal disease than the general population, due to lifestyle factors, such as community living situations, irregular sleep patterns, and sharing personal items.

Meningococcal disease strikes 1,400 to 3,000 Americans each year and is responsible for approximately 150 to 300 deaths. Adolescents and young adults account for nearly 30 percent of all cases of meningitis in the United States. In addition, approximately 100 to 125 cases of meningococcal disease occur on college campuses each year, and 5 to 15 students will die as a result.

There is a vaccination available that protects against meningococcal disease. The University of Northern Iowa (UNI) supports the Center for Disease Control (CDC) and the American College Health Association (ACHA) recommendation that all first-year college students receive the meningococcal vaccination. This vaccination offers protection against four of the five most common strains (or types) of N. meningitidis (A, C, Y, and W-135). In persons 15 to 24 years of age, 70 to 80 percent of meningitis cases are caused by potentially vaccine-preventable strains.

We are concerned about this potentially life-threatening disease. In accordance with the State of Iowa, all students must inform UNI whether they have received the vaccination against meningococcal disease. The Health Requirement Immunization Verification form in this brochure needs to be completed and sent in prior to registering for classes at UNI. *Student Health Insurance Plan (SHIP)*

The University of Northern Iowa is concerned about the potential threat the high cost of health care can pose to a student’s financial well-being. For this reason, UNI offers health insurance to students who may not have coverage. The Student Health Insurance Plan (SHIP) is a group policy administered by Wellmark Blue Cross and Blue Shield of Iowa, and the Student Dental Insurance is a group policy administered by Delta Dental of Iowa. Both plans are administered through The University of Iowa Benefits Office.
